Medication Information: PLEASE READ THIS FIRST

1. All medications, including COUGH DROPS, must be kept in the clinic. 

2. Children are not allowed to self-carry without doctor's orders. 

3. All medications (including cough drops) must have a Medication Agreement signed by a parent AND a health care provider. Medications must be in a ziplocked bag and include the drug name, correct dosage and time given. 

4. Inhalers, Epi-Pens and antihistamines for severe allergies must be on the new Colorado State Asthma and/or Allergy Plan. If your child has both, fill out BOTH forms.

5. All medications must be brought in the original container with a current expiration date.
